For in-center dialysis, entitlement begins the 1st day of the fourth month following in-center dialysis treatment starting.

Hospital admission date for transplant during dialysis waiting period

If transplant hospitalization occurred before the standard in-center entitlement date, the transplant hospitalization month becomes the initial ESRD entitlement month and the COB date recalculates from that date.

Successful kidney transplant date

Used only to calculate the 36-month ESRD entitlement end date after the transplant month.

Rules used

In-center dialysis: ESRD entitlement begins the 1st day of the fourth month following in-center dialysis treatment starting.
If transplant hospitalization occurs before that standard in-center dialysis entitlement date, ESRD entitlement begins the 1st day of the transplant hospitalization month.
Home dialysis: ESRD entitlement begins after home dialysis training begins.
Transplant with no dialysis history: entitlement begins the 1st day of the month the patient is hospitalized for transplant surgery.
ESRD Coordination of Benefits: the 30-month COB period ends on the last day of the 30th month following the initial ESRD entitlement date.
Successful kidney transplant: ESRD-based entitlement ends on the last day of the 36th month after the month of transplant.
